Care Navigation – Online Consultation Overview
Between November 2024 and December 2025, Shore Medical received a total of 143,521 online consultations for triage.
Of these, 39,501 requests were fully resolved by our Care Navigation (triage) team through advice or by directing patients to the most appropriate service.
By managing these cases without the need for a GP appointment, we were able to free up more clinical time for patients with greater medical needs.

Hypertension Webinar 2025 – Another Great Success!
Following feedback from our face-to-face Hypertension Awareness Event in September 2025, we hosted an online webinar in December 2025 to make the session more accessible and reach even more patients
We were delighted to welcome 56 patients to the webinar.
19 participants submitted blood pressure readings, making them eligible for our raffle.
5 lucky winners were drawn, and each received a £20 digital gift voucher.
Feedback was overwhelmingly positive:
91% of respondents found the discussion helpful, particularly the insights shared by Dr. Oli Smith on hypertension and cardiovascular health.
Attendees also appreciated the information presented by Esther from Health &
Care, who highlighted local services available to support wellbeing.
One attendee told us “the information given was very helpful and well visualised. It was much easier for me online rather than attending in person. Thank you very much.”

Respiratory Diagnostic Hub at St Mary’s

Our Respiratory Hub Clinics run every Monday and Thursday at St Mary’s. During these sessions, we offer Spirometry and FeNO (Fractional Exhaled Nitric Oxide) tests, which help diagnose conditions such as Asthma and COPD.
Patients can be referred to the clinic by any of our GPs or clinical team. This service is provided exclusively by Shore Medical staff for patients registered with our practice.
Urgent Request for Medication on the Urgent List
We aim to process urgent medications on the same working day whenever clinical
capacity allows.
Please order your repeat medications in advance, as urgent requests place extra
pressure on the service.
If a medication is not on the urgent list, it will be issued within 2–3 working days.
Urgent medications include:
- Previously prescribed antibiotics (if unavailable)
- Salbutamol or other reliever inhalers
- GTN spray
- Medication for abnormal heart rhythm
- Oral nitrates / Nicorandil
- Insulin
- Adrenaline pens
- Oral steroids
- Palliative care medicines
- Anticoagulants (including DOACs and Warfarin)
- Parkinson’s medications
- Anti‑epileptics (for epilepsy)
Pharmacy First
We refer approximately 400-600 patients per month to local pharmacies following triage. Only a small number of cases are returned for GP review. A clear pathway is in place for direct referral back to the GP where this is deemed appropriate.
Around 5% of those seen by the pharmacist will require routine follow up with either a GP or ANP. This means that 95 appointments are made available for every 100 pharmacy referrals, helping prioritise patients with more complex needs.
Community Pharmacies are trained and funded to see patients aged 1 and over for a wide range of common conditions, including coughs, colds, sore eyes, hay fever, pain and skin problems.


To use this service, please bring along 2 forms of ID to our reception team who will arrange access for you.
