All patients with a long-term condition should have an annual review with one of our nurses or healthcare assistants. During your long-term condition appointment, the clinician will check your blood pressure, heart rate, take a blood sample, and update your health data and lifestyle information. Reviewing your condition on an annual basis will enable you to check that you are taking the correct medications. By taking responsibility for your medical condition you can help ensure that you are having the best possible care.
