Shore Medical Offer a Nurse Lead Travel Health Service for Our Patients

This service is provided at Lilliput Surgery on a Thursday between 08.30-17.30

• You should contact your surgery for your 1^st appointment 8 weeks before departure.
• To make an appointment complete an online Klinik form or call the surgery.
• Download a Travel Health Risk Assessment for yourself and bring it to your appointment for the Nurse.
• One Form for each patient travelling and one appointment for each patient travelling.

If we cannot offer you an appointment due to the demand in service or you do not have enough time before travel we will advise you to seek private advice and vaccination.
