New Patient Registration

Parkstone Tower Practice

We normally ask new patients to allow us 48 hours to get them onto our system however we do understand that in some cases this is not possible and an urgent registration may be required, in this case please ring our reception team at the site you wish to register with.

When completing your registration form online, we will ask for a form of photo ID and an ID that shows your current address. This could be any of the following:

  • Driving licence
  • Passport
  • Utility bill
  • Bank statement
  • Local bus pass

If you are unable to complete the online registration form then please contact the surgery and they can provide you with a paper form to complete.

These fact sheets have been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Special care has been taken to ensure that information is given in clear language, and the content and style has been tested with user groups. Open the leaflets in one of the following languages:

Just visiting the area?

We are happy to see patients in need of medical attention who are visiting the area temporarily.

If routine prescriptions are required during your stay then please contact your usual GP in England, who will be able to send these electronically, using the Electronic Prescription Service, to a local pharmacy. In this situation there is no need to register with one of our Surgeries.

If you feel that you do require medical attention while staying locally then we can register you as a Temporary Resident for up to 3 months and you will be eligible for all the usual NHS services through our team. You will not have to de-register from your home practice.

If you are likely to be in the area for more than 3 months then you need to register with us or another local practice as a permanent patient. Our staff can support you in this process.

These conditions also apply to Nursing Home and Care Home residents during periods of respite or convalescence.

Please complete our Temporary Patient Registration Form and contact reception.
