New Practice Diabetes Service for Shore Medical Patients
We are delighted to introduce a new way of delivering our practice diabetes service to achieve the best possible care for our patients.
You will be invited for your annual diabetic pre-review with a health care assistant, at your own surgery. Depending on your blood test results, we will choose the most appropriate clinic for you. Well-controlled diabetic patients can attend a joint education clinic, whilst those who are less well controlled will be seen in either nurse or within secondary care. All appointments will be 30 minutes long.
Our Diabetes Clinical Team are highly-skilled and work very closely together in ensuring care is of the highest quality. We think this service will be well-received and welcome any feedback from your experiences.

What is the Aim of the Study?
- Diagnosed with Type 2 Diabetes (diagnosed within 2 years)
- Male or Female
- Aged 40-75
- Must have a smart phone
The study aims to identify effective methods to help people with T2D increase and maintain their everyday physical activity levels and start exercising regularly to benefit their physical and mental health.
