We accept new patients who will be permanently resident in our practice area for more than three months.
Our Practices are Heatherview Medical Centre, Lilliput Surgery, Wessex Road Surgery, Parkstone Tower Practice, Poole Road Medical Centre and Fernside Surgery.

To register permanently with a General Medical Practice (GP practice) you will need to complete a patient registration form, click the surgery you would like to register with below:
Patients unable to access the internet should contact the Practice in the first instance for advice on registering. We recommend you register online as this make the process much quicker, but you can obtain a paper copy at the reception of your chosen practice.
Until you are fully registered with the Practice, you will remain registered with your previous GP.
We will not be able to issue any repeat prescriptions until we have received confirmation of your past medical history and medication from your previous GP. You should ask your previous GP for a supply of medication.
Patients visiting from another area in the UK for a period of less than 3 months, and who become unwell, are entitled to be seen as a temporary resident. You will be able to make an appointment in the same way as one of our normally registered patients.
